Please print your name, street address (UPS will not deliver to a P.O. Box number), city, state, zip and your telephone number.

List the name, quantity and price of the items you wish to order.

When enclosing payment with your order we strongly recommend you contact us to confirm the total amount due including shipping and Florida sales tax (if applicable).

Payment

We accept Visa, MasterCard, Discover, American Express, check or money order in U.S. funds.

For personal checks we will withhold shipment for 10 days until funds have cleared.
